Mouse Death

Yesterday, my mouse sort of died... It's a standard 3 button optical USB mouse (blue lighted). The cord's a bit short so I use a USB extension cable. I also like to use PS/2 instead of USB. Anyway, what happened was it started not responding randomly... just not moving at all, and then moving again. I restarted a few times, reinstalling drivers, removing the extension cord, using USB instead of PS/2. It worked just fine while Windows was loading, but then once everything was loaded, it'd completely crap out on me. However, other mice work perfectly. Can anyone think of a reasoning behind this?
