The linked incident is a complete breakdown in IT procedures, which should result in everyone from the shop system administrator to the IT director getting fired. Either there were no procedures, or if there were, the procedures were not being followed, or not being audited for accuracy, not being tested for disaster recovery, or the procedures in place were incompetently specified. The only thing these people did right was keep the hard copy for a substantial time period after the initial scan, index, and storage.
